Exploring the relationship between auditory hallucinations, trauma and dissociation

Background
It is clinically imperative to better understand the relationship between trauma, auditory hallucinations and dissociation. The personal narrative of trauma has enormous significance for each individual and is also important for the clinician, who must use this information to decide on a diagnosis and treatment approach.
AimsTo better understand whether dissociation contributes in a significant way to hallucinations in individuals with and without trauma histories.
MethodThree groups of participants with auditory hallucinations were recruited, with diagnoses of: schizophrenia (without trauma) (n = 18), post-traumatic stress disorder (PTSD, n = 27) and comorbid schizophrenia and PTSD (SCZ+PTSD), n = 26). Clinician-administered measures included the PTSD Symptoms Scale Interview (PSSI-5), the Clinician-Administered Dissociative States Scale (CADSS) and the Psychotic Symptom Rating Scales (PSYRATS).
ResultsDissociative symptoms were significantly higher in participants with trauma histories (PTSD and SCZ+PTSD groups) and significantly correlated with hallucinations in trauma-exposed participants, but not in participants with schizophrenia (without trauma history). Hallucination severity was correlated with the CADSS amnesia subscale score, but depersonalisation and derealisation were not.
ConclusionsDissociation may be a mechanism in trauma-exposed individuals who hear voices, but it does not explain all hallucinatory experiences. The SCZ+PTSD group were in an intermediary position between schizophrenia and PTSD on dissociative and hallucination measures. The PTSD and SCZ+PTSD groups experienced dissociative phenomena much more frequently than the schizophrenia group, with a significant trend towards the amnesia subtype of dissociation.
Effect of Sequential Applications of Protoporphyrinogen Oxidase-Inhibiting Herbicides on Palmer Amaranth (Amaranthus palmeri) Control and Peanut Response

Two experiments were conducted in 2013 and 2014 in Florida to evaluate the effects of protoporphyrinogen oxidase (PPO)-inhibiting herbicides and single versus sequential applications on Palmer amaranth control and peanut injury. Protoporphyrinogen oxidase-inhibiting herbicides are among the last available herbicides for the POST control of acetolactate synthase (ALS)-resistant Palmer amaranth in peanut. Lactofen (219 g ai ha–1) applied 5 d after the initial application provided the highest level of Palmer amaranth control 7 and 14 d after initial application (DAIT). Delaying sequential applications of lactofen to 15 d resulted in the highest level of Palmer amaranth control 21 and 28 DAIT. Similar to Palmer amaranth control, foliar injury to peanut was often highest from lactofen applications, and by 28 DAIT lactofen treatments were the only treatments that caused foliar injury. Although no statistical difference was observed between yields of plots treated with acifluorfen (280 g ai ha–1), bentazon (560 g ai ha–1), 2,4-DB (280 g ae ha–1) alone or in combination with each other, plots treated with sequential applications of lactofen 5 or 15 DAIT produced the lowest yields. Sequential applications of lactofen applied 15 DAIT controlled Palmer amaranth more effectively than any other treatment but also caused the highest level of peanut injury. The use of sequential applications of lactofen was the most effective method for control of Palmer amaranth in this study, but did reduce peanut yield.

Use of virulence determinants and seropathotypes to distinguish high- and low-risk Escherichia coli O157 and non-O157 isolates from Europe

The presence of 10 virulence genes was examined using polymerase chain reaction (PCR) in 365 European O157 and non-O157 Escherichia coli isolates associated with verotoxin production. Strain-specific PCR data were analysed using hierarchical clustering. The resulting dendrogram clearly separated O157 from non-O157 strains. The former clustered typical high-risk seropathotype (SPT) A strains from all regions, including Sweden and Spain, which were homogenous by Cramer's V statistic, and strains with less typical O157 features mostly from Hungary. The non-O157 strains divided into a high-risk SPTB harbouring O26, O111 and O103 strains, a group pathogenic to pigs, and a group with few virulence genes other than for verotoxin. The data demonstrate SPT designation and selected PCR separated verotoxigenic E. coli of high and low risk to humans; although more virulence genes or pulsed-field gel electrophoresis will need to be included to separate high-risk strains further for epidemiological tracing.

Summary
Patients presenting with fever and rash are one of the common symptom complexes presenting in medical practice. Because of the wide range of diseases that can present with this complex, the patient presenting with fever and rash is also one of the most challenging clinical syndromes.
Although both infectious and noninfectious disease processes can present with fever and rash, infectious causes are considered here. Nevertheless, noninfectious causes such as drug reactions, systemic vasculitis, serum sickness, erythema multiforme, toxic epidermal necrolysis, and Sweet's syndrome are often in the differential diagnosis.
The approach to the patient with infectious fever and rash should begin with the appreciation that causes include common infections that are often benign, serious emergent infections that can be rapidly fatal, and unusual infections that can pose a diagnostic challenge. Key features in the history and physical can be particularly important. These include childhood diseases and immunization history, seasonal diseases, travel history and geography, exposure, sexual history, and medication usage, as well as prodromal and accompanying symptoms. Physical examination, with particular attention to the characteristics of the rash, can be key, along with vital signs to assess severity of the illness, and particular attention to meningeal signs, lymph nodes, mucus membranes, conjunctiva, and joint examination.
Dissolution Kinetics of a Simple Analogue Nuclear Waste Glass as a Function of Ph, Time and Temperature

We have measured the dissolution rate of a simple five-component borosilicate glass (Na2O, CaO, Al2O3, B2O3, SiO2) using a flow-through system. The experiments were designed to measure the dissolution rate constant over the interval pH 1 through pH 13 at 3 temperatures (25°, 50° and 70°C). Dilute buffers were used to maintain a constant pH. Analyses of solutions and solid surfaces provided information that is used to develop a kinetic model for glass dissolution.
Under all conditions we eventually observed linear dissolution kinetics. In strongly acidic solutions (pH 1 to pH 3) all components but Si were released in their stoichiometric proportions and a thick, Si-rich gel was formed. In mildly acidic to neutral solutions the gel was thinner and was both Si- and Al-rich, while the other components were released to solution in stoichiometric proportions. In mildly to strongly alkaline solutions all components were released to solution in stoichiometric proportions. By varying the flow rate at each pH we demonstrated a lack of transport control of the dissolution rate.
The dissolution rates were found to be lowest at near-neutral pH and to increase at both low and high pH. A rate equation based on transition-state theory (TST) was used to calculate dissolution rate constants and reaction order with respect to pH over two pH intervals at each temperature. At 250C between pH 1 and pH 7 based on the Si release rate the log rate constant for glass dissolution (g glass/m20d) was −0.77 and the order with respect to pH was −0.48. Between pH 7 and pH 13 the log rate constant for glass dissolution was −8.1 and the order with respect to pH was +0.51. The measured simple glass dissolution rate constants compare very well with constants estimated by fitting the same TST equation to experimental results obtained for SRL-165 glass and to dissolution rate estimates made for synthetic basaltic glasses.

A series of five successive cases of ultraviolet-induced erythroderma and keratoconjunctivitis, occurring over a one-month period and related to exposure to an improperly installed ultraviolet light are described. Although in this series, as well as others reviewed, the effects appeared self-limited, significant associated morbidity was observed.
